Fill out this form and pay $349 by credit card, HSA card, or PayPal. An independent physician will review your information and order the test for you (eligibility restrictions may apply).

Perform a simple cheek swab using the DNA sample collection kit sent to you. Then mail it back with the postage paid return label.
